| Here's how it works: |
| You or your Booster Club fundraising coordinator PRINT the order forms for the type of custom mini helmets you want to offer. CLICK HERE to print the order forms. Pass the order forms out to anyone you want - Players, parents, school faculty, students, family members, etc. While you are waiting for the order forms to be completed and turned in, you can communicate with us via e-mail or telephone to provide us with the design information for your helmets. You can e-mail digital photos of your helmet from each side or you can mail us one of your full-size helmet decals and we can replicate it in the smaller size needed for these helmets. Your players and other team supporters complete the order forms and turn them into your school or fundraising coordinator with their payment. You send the order forms and payments to Helmet Memories (our mailing address is listed on the left of this page and on the order forms). Depending on the number of helmets in your order, your school or organization receives anywhere from $2.00 to $5.00 PER HELMET! (Minimum order is only 12 helmets). |
